Administrative Officer (Elderly - TOUCH Home Care)

Main Responsibilities

  • Conduct means testing for clients at the point of enrolment
  • Determine and advise internal and external parties on the financial schemes that the client qualify for :
  • Liasie with finance to Issue payment advice to clients
  • Monitor and follow with Clients on status and schedule of payment.
  • Perform data entry and ensure accuracy of information
  • Prepare statistical reports on a regular basis for reporting, funding and audit purpose

Requirements

Main Responsibilities

  • Process referrals from AIC, hospitals or the general public. Check and route referrals to relevant centre/specialist for needs screening ; monitor turn- around time for referrals.
  • Perform acceptance or rejection of clients in IRMS system & IngoT system.
  • Collate monthly service counts from relevant departments and process monthly billing in Navision system.
  • Perform data entry, update client records and maintain proper filing system

Main Responsibilities

  • Employ creative efforts to engage and teach adults with mild to moderate intellectual disability
  • Assist in planning, developing and implementing individual education plan for adults with mild to moderate intellectual disability
  • Mentor and nurture adults with mild to moderate intellectual disability in daily living skills and community living skills
  • Monitor and evaluate their learning progress
  • Promote and coordinate enrichment programmes in areas of character development, sports and community involvement activities
  • Ensure cleanliness, good hygiene practices and safety of trainees

Care Coordinator (Elderly - TOUCH Home Care)

Main Responsibilities

- Care Coordination for Clients

  • Conduct home visits to monitor the condition of the clients; make recommendations to activate other services to support the client.
  • Refer cases to internal and external healthcare providers, mobilizing resources required to meet needs of the frail elderly.
  • Manage queries received from the public on care services provided by TOUCH Home Care, received via phone, walk in or any other platform. 
  • Provides information to caregivers and/or clients who are in need of financial, social or health care support. 

- Community Enablement

  • Assist in supporting community enablement programme.

- Social Work

  • Assist in social work cases.

Executive (Partnership - Fundraising)

Main Responsibilities
  • Responsible for securing funding and need to show track record of hitting sales/ donation targets
  • Develop insight on social issues, needs of TOUCH services and clients, in order to effectively advocate and garner support from external partners.
  • Maintain and enhance relationship with corporate partners, schools and individuals
  • Prepare and Deliver pitches/proposals for individual, corporate and school collaboration
  • Manage communication with actual or potential partners and donors
  • Research each funding prospect to develop a full, comprehensive dossier on them and to ensure that the database of donors is kept up to date.
  • Support the planning and execution of donor management

Medical Escort (Elderly - TOUCH Home Care)

Main Responsibilities

  • Escort the elderly needing assistance from their home to the hospital or polyclinic. 

  • Accompany the elderly on the vehicle.  

  • Assist the elderly throughout their hospital visit, ensuring their safety at all times.

  • Adhere to the Standard Operating Procedures (SOP) at all time. 

  • Inform the co-ordinator of any abnormality of the client.

  • Physically able to handle elderly on a wheelchair. 

  • Assist in Meals-On-Wheels delivery as required. 

  • Assist in other ad-hoc activities.

Main Responsibilities

  • Source and provide relevant evidence-based information to support TOUCH’s programmes and services
  • Work with programme teams to design and develop evidence-based programmes and activities
  • Develop monitoring indicators and scales for key programmes to prepare for programme evaluation (both internal and external)
  • Co-ordinate and manage programme evaluation projects with external programme evaluators
  • Collaborate with service teams to conduct research studies
  • Prepare reports and presents results of research studies, needs assessments and programme evaluation projects
  • Present research and evaluation findings through reports, newsletters, presentation and others
  • Develop and provide information and training on research and evaluation
