Indeed, home is the best place to be in. With more than 25 years of experience in geriatric care, THC is the first home care agency in Singapore to provide integrated and holistic home-based services that support the frail and homebound elderly and their caregivers.
Our elderly clients are served by a trans-disciplinary team of health and home care professionals as well as volunteer befrienders. Our customised services include home medical, meal delivery, housekeeping, personal hygiene services, and transport services to hospitals for appointments. THC operates out of three centres located in Ang Mo Kio, Jurong West and Toa Payoh.
Our therapists and nurses, who are disability assessors accredited by MOH, can also conduct assessment on an individual’s functional status at home to determine eligibility for severe disability schemes, such as Careshield, Eldershield and IDAPE.
In 2017, THC was honoured with the Best Home Care Operator Award at the 5th Asia Pacific Eldercare Innovation Awards for providing outstanding health and social services that promote ageing-in-place among seniors. Our staff have also been consistently recognised at national-level eldercare awards.
As part of our efforts to promote greater ownership in the elderly to manage their own health and reduce reliance on care services, the Reablement Project was launched in 2022.
