Job Summary:
As a Social Worker/Counsellor, you will be responsible for organizing and conducting outreach to increase mental health awareness. You will support clients who are at risk of mental health issues by providing mental health screening and assessments, and by linking them to the necessary support and resources. You will continue to monitor these clients in the community.
You will also be involved in data collection and evaluation of the CREST programme.
Principle Duties & Responsibilities:
Programme Development and Implementation
- Provide care coordination and service linkage to clients with or who are at risk of mental health concerns
- Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
- Conduct assessment (i.e. Mental Health needs, behavioural, developmental, risk, safety)
- Provide counselling, case management, and psychosocial interventions
- Conduct active monitoring and maintenance of clients
Administration and Staff Development
- Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance with the CREST-Y programme requirement
- Keep and maintain client and programme data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines
- Support in the development of CREST service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up)
- Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme
- Network with community partners for collaboration/co-management
Other Duties
- Participate in relevant meetings with key stakeholders, including AIC to share key learning points where required
Requirements:
- Minimum Degree, or post-graduate diploma, ideally in Social Work, Counselling, Psychology, or other related fields.
