FFL Consent Clauses and

Terms of Consent

CONSENT FOR COLLECTION, USE AND DISCLOSURE OF PERSONAL INFORMATION
 

  1. I understand that the Government of Singapore, as represented by the Ministry of Social and Family Development (“MSF”), Families for Life (“FFL”), and the Relevant Organisations require Personal Information belonging to me and my spouse/spouse-to-be for the following purposes (as the case may be):

    (a) to contact and/or refer me and my spouse/spouse-to-be to participate in appropriate programmes, services, and activities provided by FFL and/or the Relevant Organisations to support us in our marriage

    (b) to provide me and my spouse/spouse-to-be with programmes, services, and activities to support us in our marriage;

    (c) to include me and my spouse/spouse-to-be in the contact lists belonging to FFL to:

        (i) provide me and my spouse/spouse-to-be with information and materials relating to family resilience;

        (ii) provide me and my spouse/spouse-to-be with regular updates on engagement events, activities, and programmes organised by FFL and/or the Relevant Organisations; and

        (iii) invite me and/or my spouse/spouse-to-be to contribute, share opinions and/or provide feedback on the programme(s) I and/or my spouse/spouse-to-be attend; and

    (d) for data-analysis, evaluation, and policy-making.

  2. I further understand that:

    (a) where appropriate, my and my spouse/spouse-to-be’s Personal Information may be shared to serve me and my spouse/spouse-to-be in the most efficient and effective way unless such sharing is prohibited by law; and

    (b) my and my spouse/spouse-to-be’s Personal Information will not be shared with non-participating agencies and organisations.

  3. I consent and agree that FFL and the Relevant Organisations may collect, use and disclose my Personal Information for the purposes stated in Paragraph 1.

  4. I confirm that I have informed my spouse/spouse-to-be that his/her Personal Information will be provided to FFL and the Relevant Organisations for the purposes stated in Paragraph 1.

  5. I confirm that I have obtained the consent of my spouse/spouse-to-be for FFL and the Relevant Organisations to collect, use and disclose his/her Personal Information for the purposes stated in Paragraph 1.

  6. My consent remains valid until I withdraw it in writing. I accept that it will take up to 14 working days from the date of receipt before the withdrawal of consent takes place.

  7. I confirm that I have informed my spouse/spouse-to-be that his/her consent remains valid until he/she withdraws it in writing. I also confirm that my spouse/spouse-to-be accepts that it will take up to 14 working days from the date of receipt before the withdrawal of consent takes place.

  8. I have read and understood this consent form fully, including the Terms of Consent.

  9. I confirm that I and my spouse/spouse-to-be understand that if we have any questions relating to the collection, use or disclosure of our Personal Information, including how we may access and correct our Personal Information or withdraw our consent to the collection, use or disclosure of our Personal Information, we may submit our queries via email to ask-familiesforlife@msf.gov.sg.

  10. I declare that the information that I have provided is accurate as at the time I submit this form.
     

Terms of Consent
 

  1. I understand and agree that these terms used in the consent form have the following definitions:

    (a) “Personal Information” includes the following but is not limited to:

          (i) Demographic information (e.g. bio-data comprising name, identification number, email address, postal code, mobile number, sex, date of birth, ethnicity, residential status, marital status);

          (ii) Education information (e.g. highest educational attainment);         

          (iii) Gross monthly household income (e.g. my and my spouse’s combined income bracket);        

          (iv) Housing information (e.g. housing type); and    

          (v) Responses to feedback forms and evaluation questionnaires.

    (b) “Relevant Organisations” refer to any organisation or any other company which has been authorised by FFL, to collect, use or disclose Personal Information for the purposes stated in Paragraph 1. New organisations and/or companies may be included from time to time.

  2. This consent shall be governed and construed in accordance with the laws of the Republic of Singapore.
