A dedicated point of contact for seniors to receive a holistic long term care plan that allows them to enjoy improved health, social, and care services in the community.
Enabling A Co-ordinated Care Journey for Seniors
By 2030, 1 in 4 citizens will be aged 65 and above. TOUCH is working with the Ministry of Health (MOH) and the Agency of Integrated Care (AIC) to support seniors to age in the community by making care services more accessible.
These services aim to assist seniors above 60 years old to:
- Cope with their health changes through a customised holistic long-term care plan
- Be connected and active through active ageing programmes
- Access relevant care services to allow them to age in their homes
