Introducing the Heart of Community Fund
TOUCH Community Services is once again inviting you to be part of the Heart of Community Fund — a dedicated fund that brings hope, care, and support to individuals and families who need it most. Every gift helps sustain and grow TOUCH’s programmes and services across Singapore, ensuring that no one is left behind.
This year, the fund will support some 216,000 individuals in our care, including 13,322 seniors, 5,496 caregivers, 1,905 persons with special needs and their families, and 48,899 children, youths, parents and educators. Funds from The Heart of Community Fund will also support outreach programmes that promote cyber wellness and mental health, reaching over 109,000 individuals in the community.

From Hesitation to Hope: Farhan’s Journey of Growth Through ADAPT
Once shy and unsure of himself, 18-year-old Farhan struggled to adjust to a new environment, finding it hard to make friends and step out of his comfort zone. Life felt uncertain, and even simple social connections were a challenge. Things began to shift when Farhan’s class was selected to join ADAPT, a youth mentoring programme by Aptitude, under TOUCH Youth Enablement (TYE). Designed to build resilience and confidence while helping youths uncover their strengths, ADAPT provided Farhan the spark he needed. He found a safe space to grow while been supported by mentors who believed in him and guided him to discover his strengths.
With that support, Farhan began to change. He grew in confidence, built meaningful friendships, and even stepped up as an emcee for a recent carnival, something he once never imagined possible. Despite setbacks, he persevered with the encouragement of his mentors, emerging stronger and more resilient. Today, Farhan faces the future with confidence and purpose. His growth is a powerful reminder that with the right support, hesitation can be transformed into hope.

Turning Fear into Empowerment

Once fiercely independent, Mdm Yeo’s world began to shrink after age and illness took a toll on her body. Living alone and struggling with mobility after multiple surgeries and a fall, even simple tasks like buying groceries became impossible. She had to rely on others for daily needs and that weighed heavily on her as she longed to regain control over her own life.
Everything began to change when she was introduced to TOUCH’s Reablement Programme. Instead of doing things for her, the team walked alongside her to guide her, encourage and equip her with the strength and confidence to try again. Step by step, Mdm Yeo regained her mobility, overcame her fears, and found the courage to leave her flat on her own. Today, she is once again able to run errands independently, reconnect with the world around her, and live with dignity and confidence.
