About the Activity
TOUCH Community Case Management (CCMS) provides a one-stop service to help seniors living in Toa Payoh and Novena to lead independent and dignified lives.
Through regular home visits and medication reminders, volunteers directly contribute to seniors’ overall health and quality of life, enabling them to age gracefully and with dignity.
Schedule & Commitment
Flexible schedule between Monday to Sunday, with a commitment of at least 1 home visit per week for at least 6 months.

Requirements:
- 21 years old and above
- Passionate about supporting seniors to age-in-place in their community
- Patient and understanding when working with seniors with memory loss
- Committed to making a positive difference in the lives of seniors
- Comfortable in providing practical support to seniors in their homes
Your primary role involves:
- Conduct regular home visits to remind and supervise senior taking their medication
- Promptly informing the social worker of any concerns regarding the senior's health or safety
